Wellness Program Agreement (Military)

1. Special Discounts Overview

Veterans
Active Military Personnel

Valid identification or proof of service is required to receive discounts.

Discount Structure

15% off all non-member services and add-ons.
Additional 5% off on wellness program fees and member rates for services and add-ons.
Exclusive Packages designed specifically for eligible individuals.

2. Wellness Program Tiers with Additional Discounts

We have adjusted our wellness program tiers to provide extra savings for eligible individuals.

Dosha Air Wellness Program (Military)

Monthly Fee: $80.75 (Standard $85 - 5% discount)

Included Services:

One 60-minute hands on massage per month.
Total Appointment Time: 80 minutes.

Additional Benefits:

20% off standard rates for extra massages (Standard 15% + Additional 5%).
Rollover: Unlimited rollover *Subject to change as a result of operating as a solo practitioner. A minimum of 3 months/90 days' notice will be provided prior to any adjustments.*

Late Cancellation Fee: 50% of the scheduled service price.
No-Show Fee: 100% of the scheduled service price.
Wellness Program Reactivation Fee: $25 (if canceled within 3 months of signing up).

Dosha Ether Wellness Program (Military)

Monthly Fee: $114 (Standard $120 - 5% discount)

Included Services:

One 90-minute massage per month.
Total Appointment Time: 110 minutes.

Additional Benefits:

25% off standard rates for extra massages (Standard 20% + Additional 5%).
Complimentary Add-On per month.
Rollover: Unlimited rollover *Subject to change as a result of operating as a solo practitioner. A minimum of 3 months/90 days' notice will be provided prior to any adjustments.*

Monthly Fee: $152 (Standard $160 - 5% discount)

Included Services:

Two 60-minute massages per month.
Total Appointment Time per Session: 80 minutes (if you use both of these for a 2 hour service the total time then becomes 120 mins hands on + 20 mins for checkout, recommendations, and table flipping).

Additional Benefits:

30% off standard rates for extra massages (Standard 25% + Additional 5%).
Two Complimentary Add-Ons per month.
Rollover: Unlimited rollover *Subject to change as a result of operating as a solo practitioner. A minimum of 3 months/90 days' notice will be provided prior to any adjustments.*

Monthly Fee: $199.50 (Standard $210 - 5% discount)

Included Services:

Two 90-minute massages per month.
Total Appointment Time per Session: 90 minutes (if you use both of these for a 3 hour service the total time then becomes 180 mins hands on + 20 mins for checkout, recommendations, and table flipping).

Additional Benefits:

35% off standard rates for extra massages (Standard 30% + Additional 5%).
Unlimited Complimentary Add-Ons.
Rollover: Unlimited rollover *Subject to change as a result of operating as a solo practitioner. A minimum of 3 months/90 days' notice will be provided prior to any adjustments.*

3. Adjusted Non-Member Pricing

Standard Services

Eligible individuals receive 15% off all standard service rates.

30-Minute Massage

Standard Price: $60

Discounted Price: $60 - 15% = $51

60-Minute Massage

Standard Price: $100
Discounted Price: $100 - 15% = $85

90-Minute Massage

Standard Price: $140
Discounted Price: $140 - 15% = $119

4. Operational Considerations

Verification Process

Proof of Service: Acceptable forms include military ID, veteran ID, department ID, or other official documentation.
Renewal of Eligibility: Verification required annually to continue receiving discounts.

Staff Training

Awareness: We will train future staff to recognize eligible IDs and apply discounts correctly.
Respect and Confidentiality: We will ensure all interactions are handled with the utmost respect and privacy.

5. Marketing and Promotion

Community Outreach

We will look to partner with local military bases, fire departments, and police stations to promote services.
We offer on-site chair massages during special events.

Referral Program

We encourage you to refer eligible individuals which get you additional loyalty points or discounts.

Special Events

Host appreciation days with extra perks for service members, such as complimentary add-ons or exclusive workshops.

6. Client Experience Enhancement

Personalized Services

Consultations: We offer specialized consultations to address stress, physical strain, and other common issues faced by service members. 30-Minute Consultation: Price: $40.
Flexible Scheduling: We will try to provide priority booking options to accommodate varying schedules, especially for active-duty personnel.

Wellness Resources

Provide educational materials on stress management, physical health, and self-care practices.

7. Additional Benefits

Wellness Program Pause Options

We will allow wellness programs to be paused for longer durations without additional fees to accommodate deployments or extended duties.

8. Wellness Program Cancellation and Freeze Fees

Wellness Program Cancellation

Notice Requirement:

30 days email notice is required to cancel a wellness program.

Early Termination Fee:

A fee of $50 applies if canceling within the first 3 months of wellness program.

Reactivation Fee:

A $25 fee applies to reactivate a canceled wellness program if canceling within the first 3 months of wellness program.

Wellness Program Freeze

Allowed Freeze Duration:

Varies by tier (e.g., up to 1 month per year for Tier 1).

Freeze Fee:

No fee for freezing within allowed duration.

Extended Freeze:

50% of the current tier rate per month fee for freezes beyond the allowed period.

A. Returned Payment Fees

Insufficient Funds Fee:

A fee of $35 will be charged for returned checks or declined automatic payments.

Responsibility:

Clients are responsible for ensuring sufficient funds and updating payment information.

B. Other Fees

Credit Card Processing Fee:

No additional fees for credit card payments.

Gift Card Replacement Fee:

$5 fee to replace lost or stolen gift cards.

Product Return Restocking Fee:

10% restocking fee applies to returned retail products, unless defective.

9. Conclusion

By incorporating these discounts and benefits into our business model, Dosha Bodywork & Wellness LLC aims to honor and support the well-being of those who serve our communities and nation. We are committed to providing accessible, high-quality wellness services tailored to the unique needs of veterans, active military personnel, firefighters, and law enforcement officers.

In most cases, only one form of verification document is required for submission, unless you have a special situation (such as if your name has changed since your military service).
Other documents that clearly show your first and last name, valid dates, branch, and/or status may also be accepted.
If the name on your document(s) does not match your current legal name (such as Marriage or Name Change) you must submit additional documentation (Change of Name, Marriage Certificate, etc.) with your form, or acquire a new copy of the document with your current name for us to verify your military affiliation.
Any sensitive information such as Social Security Number, Military ID Number, etc. should be omitted, covered up, or blacked out when submitting.
The information provided is used solely for verifying military status and is destroyed after verification.
Acceptable file formats include .bmp, .gif, .jpg, .png, or .pdf.
